We've launched four new Shingles vaccination indicators in Hippo, covering both doses of Shingrix across immunocompetent and immunocompromised patient groups.
The indicators are designed to help practices work through their eligible populations systematically: distinguishing between cohorts, tracking first and second dose completion separately, and avoiding unnecessary recalls for patients already appropriately vaccinated.

The indicators cover:
First and second dose tracking for immunocompetent patients, based on age and vaccination history
First and second dose tracking for immunocompromised patients identified through clinical review
Automatic exclusion of patients with contraindications, recent declines, or prior Zostavax vaccination where applicable
These are positive care delivery indicators:
Complete = the relevant Shingrix dose has been recorded
Not complete = the patient appears eligible and may need inviting
1๏ธโฃ HIPPOSHING_IC_01 - Shingrix First Dose (Immunocompetent)
Identifies immunocompetent patients due their first dose of Shingrix.
Eligible patients
Patients must meet one of the following age criteria:
Aged 65 or over and born after 1 September 1958, or
Aged 70โ79 with no record of a shingles vaccine prior to 31 October 2024
How completion is determined
The indicator is marked complete once a first Shingrix dose is recorded by any healthcare provider. Patients without a recorded first dose will be marked as eligible.
Exclusions
Patients will not be eligible if they:
Have a contraindication to the shingles vaccine
Have declined a shingles vaccine in the last 12 months
Have received Zostavax on or after 1 January 2013
Have any shingles vaccination recorded before 31 October 2024 (assumed Zostavax)
Have a "requires vaccination against shingles" code. These patients sit within SH_REQSHV_1 and SH_REQSHV_2
Eligibility window
Campaign runs until 31 August 2028.
2๏ธโฃ HIPPOSHING_IC_02 - Shingrix Second Dose (Immunocompetent)
Identifies immunocompetent patients due their second dose of Shingrix.
Eligible patients
Patients must meet one of the following age criteria:
Aged 65 or over and born after 1 September 1958, or
Aged 70โ80 with no record of a shingles vaccine prior to 31 October 2024
Patients must also have received their first Shingrix dose at least 6 months ago.
How completion is determined
The indicator is marked complete once a second Shingrix dose is recorded by any healthcare provider. Patients without a recorded second dose will appear in recall lists.
Exclusions
Patients will not be eligible if they:
Have a contraindication to the shingles vaccine
Have declined a shingles vaccine in the last 12 months
Have received Zostavax on or after 1 January 2013
Have any shingles vaccination recorded before 31 October 2024 (assumed Zostavax)
Have a "requires vaccination against shingles" code. These patients sit within SH_REQSHV_1 and SH_REQSHV_2

4๏ธโฃ HIPPOSHING_CX_02 - Shingrix Second Dose (Immunocompromised)
Identifies immunocompromised patients due their second dose of Shingrix following clinical review.
Eligible patients
Aged 18 or over
Has a "requires vaccination against shingles" code dated on or after 1 September 2023
Has received their first Shingrix dose at least 8 weeks ago
How completion is determined
The indicator is marked complete once a second Shingrix dose is recorded by any healthcare provider. Patients without a recorded second dose will appear in recall lists.
Exclusions
Patients will not appear if they:
Have a contraindication to the shingles vaccine
Have declined a shingles vaccine in the last 12 months
Have received Zostavax on or after 1 January 2013
Have any shingles vaccination recorded before 31 October 2024 (assumed Zostavax)
Note: Practices do not need to add a second โrequires vaccination against shinglesโ code to the patientโs record: if they have one โrequires vaccination against shinglesโ code and have had one Shingrix vaccine, then they will automatically be invited for a second Shingrix vaccine 8 weeks later.
๐ Coding Notes
Dose tracking First and second doses are tracked separately across all four indicators. Vaccinations administered outside the practice are included where coded appropriately.
Exclusion logic
For immunocompetent patients (SHING_1 / SHING_2): decline codes expire after 12 months. Patients may re-enter eligibility automatically once this window passes
Any recorded decline code within the last 12 months will exclude the patient
Patients vaccinated with Zostavax on or after 1 January 2013 are considered appropriately vaccinated and will not appear
Patients with a "requires vaccination against shingles" code are excluded from SHING_1 and SHING_2, and should be managed via SH_REQSHV_1 and SH_REQSHV_2
๐ก Things to Be Aware of
The Zostavax exclusion logic is worth understanding before running recalls.
Patients who received any shingles vaccination before 31 October 2024 are excluded on the assumption they were vaccinated under the previous Zostavax programme (while Zostavax was still in stock).
Patients who received Zostavax on or after 1 January 2013 are also excluded, as they will already be protected.
Patients who received Zostavax before 1 January 2013 however are included because their protection from the vaccine will have waned.
The immunocompromised cohort (SH_REQSHV_1 / SH_REQSHV_2) requires a "requires vaccination against shingles" code dated on or after 1 September 2023 before a patient will appear; this won't happen automatically and relies on practices having reviewed and coded their potentially immunosuppressed patients.
