As part of your recall campaigns, Hippo may send patients a short online questionnaire to collect basic health information. This helps keep records up to date and ensures patients get the right care without needing to come into the practice unnecessarily.
🧠 How it works
The questionnaire is sent via a secure Typeform link. Patients will only see questions that are relevant or missing from their medical record — they won’t be asked to answer everything.

🎂 Date of Birth
“Please confirm your date of birth”
📏 Height
“Please enter your height in cm”
⚖️ Weight
“Please enter your weight in kg”
💓 Blood Pressure
“Are you able to provide a blood pressure reading?”
If yes, they’ll then see:
“Please enter your systolic blood pressure reading (top number)”
“Now enter your diastolic blood pressure (bottom number)”
If no, it’ll move on to the next question.
🚬 Smoking Status
“Are you a smoker / have you ever smoked?”
Options: Yes / No, but I used to / No
If they answer “Yes”, they’ll be asked:
“How many cigarettes do you smoke per day?”
They’ll need to type a number, or “0” if they no longer smoke (or clicked this option by accident!).
If they answer “No” or “No, but I used to smoke”, this follow-up question will be skipped automatically.
🌫️ Second-hand Smoke Exposure
“Do you have regular exposure to second-hand smoke?”
🍷 Alcohol Consumption
“Do you drink alcohol? If so, how many units would you consume on the average week?”
Includes a link to the DrinkAware calculator
🩺 Diabetes Education Programme (if applicable)
“Please indicate whether you would like to sign up for the Diabetes Educational Programme”
✅ Good to know
Most patients will only be asked 2–4 questions depending on what’s missing.
It usually takes under 3 minutes to complete.
Responses are collected once submitted and you can review these in the 'Unapproved Basic Health Data' Worklist.
Once your team have reviewed the information, each piece of data will be coded back into the patient's EHR.
Patients can skip the questionnaire or call the practice instead if they prefer.
