🧪New CQC DOAC Monitoring Indicators

We’ve added new CQC indicators for DOAC monitoring to make it easier to identify what’s missing for each patient and ensure reviews are completed safely and accurately.


🔍 What’s new

The DOAC indicators have been split into separate parts so you can now see exactly which step of the monitoring process still needs to be done.

1️⃣ CQCDOAC001–004 — DOAC monitoring

These indicators cover all required review intervals (annual, 6-month, and 3-month checks, depending on kidney function).

Each now includes A and B versions:

Indicator Type

What it checks

Details

A indicators

Blood test (serum creatinine)

Confirms that a recent serum creatinine result is recorded for the patient.

B indicators

Creatinine clearance (CrCl)

Confirms that a CrCl has been calculated — this depends on having both a recent creatinine result and a weight.
⚠️ Patients won’t appear on the B indicator list until both results are recorded.

These indicators are:

  • CQCDOAC001 / 001A / 001B – Standard DOAC monitoring

  • CQCDOAC002 / 002A / 002B – Annual CrCl review

  • CQCDOAC003 / 003A / 003B – 3-month CrCl review (for CKD4/5)

  • CQCDOAC004 / 004A / 004B – 6-month CrCl review (for CKD3)


2️⃣ CQCDOAC000 — Weight recording

  • Checks that the patient has a weight recorded in the last 12 months.

  • Weight is required for accurate CrCl calculations — without it, CrCl (B) indicators will not appear or show as complete.


💡 Why this matters

These changes make it easier to:

  • See which part of the DOAC review still needs to be completed (blood test, weight, or CrCl).

  • Ensure that creatinine clearance values are accurate and based on up-to-date data.

  • Stay compliant with CQC requirements for safe medicines monitoring.
